Download the Declaration of Finances form and fill it out. Also, you will need to submit bank statement to verify the amounts listed in your Declaration of Finance.

 

Download the Transfer-In Form and fill in the top portion. Take the form to the office which produces your I-20 and ask them to complete the form.
